Permanently Disabled Voters

The Permanently Disabled Voters List is a list of voters who can no longer get to the polling place to cast their vote on Election Day and, for that reason, recieve an Absentee Ballot Applicaton in the mail each year.

In order to have your name placed on the Permanently Disabled Voters list, you must send a written letter from your Doctor to the City Clerk's Office (form below).

Please keep in mind the list only provides that an Absentee Ballot Application will be sent to the voter. It is the voter's responsibility to complete the application each year in order to recieve an Absentee Ballot for any or all elections.
